Release of Liability

In consideration of being allowed to participate in any way in the Atlanta Indoor Volleyball Club (AIVC) programs and related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify, and hold harmless the AIVC, its officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (collectively the "releasees"), with respect to any and all injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the releasees or otherwise, to the fullest extent permitted by law.
2. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist.
3. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my participation, to the fullest extent permitted by law.
4. I willingly agree to comply with the stated and customary terms and conditions for participation. If I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately.
5. I am aware and understand that I may require emergency medical care as a result of participating in the activity. I authorize AIVC to provide or have provided to me such emergency care as it deems necessary, at my expense, and I release AIVC from any liability for these costs.

This agreement is governed by and interpreted in accordance with the laws of the state of Georgia, without regard to its conflict of law principles. Any litigation or other legal action that involves the parties to this agreement must be brought exclusively within the state of Georgia. The parties hereby irrevocably agree to such exclusive jurisdiction.

If any provision of this agreement is determined by a court of competent jurisdiction to be invalid, illegal or unenforceable in any respect, such determination shall not impair or affect the validity, legality or enforceability of the remaining provisions.

Photo Release

I hereby grant permission to the AIVC to use photographs and/or video of me taken during league activities in publications, news releases, online, and in other communications related to the mission of the AIVC.
